I like how all of these bugs are results of big, open worlds and/or sanbox gameplay... except the HL2 one.

The Oblivion Knights of the Nine "issue" is completely mis-diagnosed. The reason that the knight stopped attacking the player was because he yielded, cancelling the combat, but the other knights didn't stop.

I'm surprised they didn't include the Oblivion classic: attack any number of enemies who don't have ranged weapons, jump on rock, kill them all with a bow while they run ceaselessly into the rock without being able to get on top of it or reach you with their swords.

Thief DS's AI bug that reverted the state from expert to normal after loading saves should be there somewhere.
I know this really doesn't count as a bug, but as far as AI quirks go; continuously trying (and failing) to keep my animal totem from eating it's own feces in Black and White was endlessly frustrating.
