There's a patch that fixes some issues, but you may still have problems. Don't ever use the saves the game makes; use your own.

I would recommend not using any saves at all mid-mission and ironmanning the missions. They aren't very long and I think even with the latest patch sometimes reloading causes weird things to happen.

Nah, that's not necessary. Just make a manual save (not a quicksave) every time you reach a checkpoint and you should be good as long as you load from that save and only that save. Probably be a good idea to have two saves- one for your safe house, the other for during missions.

There aren't quick saves per se. Just checkpoint saves, and there are certainly still missions where loading a checkpoint causes anomalies.
What exactly happens is all over the place though. The one that springs to mind first is the stealth mission in Saudi, where if you reload a save it becomes considerably easier since half the enemies vanish. I seem to remember there being a similar issue with the stealth mission in Taipei. The mission in Rome where you investigate the warehouse can also muck up, but that one's short enough to redoing it isn't much of a problem.

See that's just it. I only ever had a problem if I loaded a save the game made. If I loaded a manual save, the game worked fine.

This. Personally, I never had any problems with saves (auto or otherwise). YMMV.

It was probably fixed in the patch, as I haven't played it through all the way since then. At release, if you reloaded a save in a mission wacky shit would happen like all of the enemy NPC's disappearing from the area you were in.

That only happened with saves the game made, before the patch. It happened after, too, though.
