No idea; obviously the developers wanted him to pop up so as to give the player an unexpected shock and to see the depths of Karras' madness. The simplest explanation for how he pops up would probably be that the boy was in a compartment in the wall that had a mechanism connected to the scripture that opened once the book was opened, and would of closed by the time Garrett stopped reading it.
The Golden Cherub makes me, to this day, put off the scripture bit of angelwatch till last.
What really gets me is how the servants of mechanists arn't phased by this disturbing creation. Although the developers probably did this to save on time it is nonetheless unsettling.

Before reading the scripture, make sure the door to the office is open. After reading the scripture, select the key to that door. Then run. Don't stop, don't look, just run. As soon as you're out, turn around and lock the door. Golden Cherub? What Golden Cherub? :ebil:

The idea that he is the design for the occupant of the new world also makes sense to me. As Karras saw things it was his destiny to bring about the Builder's perfect world. So the occupants would need to be variants on the Builder's design, but, as Karras saw it, perfected.
So you're telling me that he's not supposed to be Father Karras's little altar boy? Does that make me sick for assuming that? :o
