I'm interested in hearing what people find most irritating in games- general concepts or gameplay mechanics.
Personally, I hate when an enemy is revealed through a cutscene, while the enemy is in the same room as me, so I can only wait as the camera slowly flies back to my head before I can do anything.
I also strongly dislike games where enemies don't react to being shot. A lot of games have enemies that require a lot of ammo to bring down, but it's made worse when they don't flinch or otherwise respond to having bullet holes through them.

I hate NPC babysitting missions which can be more or less worse depending on the AI. Every time I have to protect someone from bullet fire or something else, I feel like choking the guy responsible for that whole ordeal.
I can deal with failing an ingame objective by dying myself or not figuring out what to do. But failing said objective because a dummy of an NPC gets shot is just not cool.

Unforgiving and arbitrary save systems.
I have things going on in real life, developers. Don't think that you can make me replay a dull-as-rocks half-hour stretch because, earlier, I had to get up and go do something else. Your game is not so incredible an experience that it can't be interrupted.

That last one reminds me- unskippable cutscenes, especially in games where you can't save right after the cutscene. Although I suppose the horse has been beaten, killed, and buried by this point.

Probably not the most irritating, but repetitious and time-consuming mini-games rank pretty high. Playing the pipe puzzle in Bioshock for the 200th time wasn't even close to fun.

Countdown timers. I don't care if it's a countdown to exit a building before it blows, or to kill a certain enemy, or shit like the life diminish in Soul Reaver 2 or the spirit meter in MotB - I hate it with a passion. I like to take my time to explore my surroundings, and even if there's not much to explore, I hate not having the option to proceed at my own pace.

And then there's plenty of shit in MMOs, but there's little point going through that since it's all intentionally there to make games less fun and more like a long, boring job. I assume we're not talking about
