Because I'm playing Risen at the moment, the one thing on my current hate list is when core game mechanics are fundamentally broken and the game forces you to make use of these same broken mechanics.
In Risen the most obvious broken mechanic is the control system, particularly collision detection when jumping off cliffs. Actually, jumping in general sucks ass, but slowly edging your way to the side of a cliff so you can line up your fall to a lower section below only to stutter for half a second with your arms out wide as if you are attempting to fly and then BOUNCING off the side of the cliff, missing the mark you were aiming for and falling to your death in the ravine below is, well.... :mad::mad::mad:
And it wouldn't be so bad except the developers put in quite a few cliff hoping scenarios where this broken mechanic MUST be dealt with. Did they just think I wouldn't notice this obvious flaw in their system? Did they even play their own game before shipping it?? (A question I ask myself far more often than should be allowed)

Ack, I hate that. inFamous is a particularly bad offender here. Every time you drain the health of someone, you have to repeatedly tap a button.
Wet also does it for opening doors. What's worse is there's absolutely no reason to delay the player with this mechanic in Wet, as the player's not at risk while doing it at any point in the game.
My personal bugbear is decorative doors. If you're going to put a door in your level, make damn sure to put something behind that door.
Oh yeah, and another one, peculiar to Bioware titles: the tardis effect, where the inside of a building is noticeably larger than the exterior would lead you to believe. Of course, this is mainly because of engine restrictions, but leads on to another one: building interiors that have to be loaded. I'm looking at you Bethesda. It's nice to be able to take cover in buildings, and "interact" with the world outside. Loading interiors seperately immediately shatters immersion.

Long load times that make me afraid to die or move to another area because I know I'll have to wait for so long. Games like Gothic 3 and NWN2 are big offenders here.

Yes, I am referring to that. It appears at the end of a cutscene, and the first time I got there, I died because I didn't realise it was suddenly interactive.
And the fact that the key you have to his to not die is 'X', and "Hit X to not die" is one of Yahtzee's catchphrases, certainly implies that it's something of a parody to me.

Godyes. Playing Saints Row 2 right now and the best advice I can give to anyone thinking about eventually playing this: DO NOT LET YOUR TEAMMATES GET THEIR HANDS ON AN RPG LAUNCHER! They do not seem to have any concept of how far away the enemy they're shooting at is from themselves, or you. There was this one mission where I had to outrun the cops but my idiot teammate kept popping out the sidewindow and shooting rockets at the copcars when they were bumper to bumper behind us, naturally blowing up our car in the process. After 4 retries I got out of the car before we started our journey and put a bullet in my teammates head. Then got back in the car and drove on, finishing the mission in one go.

Having to return to the start point of a mission/challenge after you failed it - Burnout Paradise is one of the more recent culprits, I'm pleased to say GTA has long since learned the lesson.

This is also annoying (makes me want to hit the crap out of the game producers): Agressive computer help.
Many games I've tried to play but couldn't go over the first level because I had to go trough tutorials, some stupid animated goblin popped up and wanted to explain something to me or simply the items and moves started to explain themselves when picked up or used. And there's no way to make them shut up and go on with the game! Mostly this shit comes with second editions of games, which were great at first, but suck at 2.,3.,... (For example Dungeon Keeper or Simon the sourcerer)

Unmovable cameras in strategy games.
I've been playing Metal Gear Acid 2, and it's quite good, but there are times when you need to see a bit outside of the camera normal range, or things are obstructed, and you can't move it because it's locked on the character. So you end up sniping at enemies that are not on screen, etc.
But yeah, previously mentioned save-system issues bug me more than anything.
