In Vice City there would be a taxi waiting near the hospital or police station if you died / got busted on a mission, but San Andreas ditched it... so... maybe they learned their lesson, then forgot it again.

One that really irks me is games where you can only save at certain points and checkpoints. It's just so frustrating having to get to a certain area or beat a certain boss or *shudder even backtrack all the way to a save point just to save your game. I should be able to save when I want and wherever I want.

Nope, what Ulu is talking about isn't there. You can check your cellphone right after you get fail the mission to redo it right away.

I'd like to submit my latest pet-peeve to this list.
Exaggerated gesticulating during cut-scenes … Ugh!
Having the "Big-bad-crazy-guy" pacing furiously and waving his arms about during his "This-is-why-I-must-destroy-it-all" speech is clichéd and comical. Games with half-decent character models and half-decent voice actors need not resort to this … gimmick … ever.

Oh - this is a minor one, but forced subtitles. For whatever reasons, there are some games that have subtitles forced on despite being in English. If I speak the language, I don't need subtitles!

Why is it that the moment you lose control of your character, they invariably do something stupid? Generally in a ham-fisted way of advancing the plot.

Mentioned earlier, but the inability to save your game wherever you want. There'd better be a REALLY good gameplay reason for it, and max 5 minutes between savable locations. Many people wouldn't even be that generous!
Also, games that do not let you re-configure your controls. Fortunately I haven't come across one of those for many years now, but I'm guessing there may be some still out there...
