A thief-like game? Try Doom 3, bwahaha! So much darkness you can't see diddly-squat, and the atmosphere will blow you away. Play while alone in the dark with headphones and you'll be able to give three urine samples in a row! And the stealth element? Only a super-extremo master thief can avoid the enemies in that game--it's impossible.
Heh. Honestly, Thief is rather one-of-a-kind. No other game can be more Thief-like than Thief itself. Other games that try to include stealth as an element don't end up working well because they try to include so many other features at once and they end up being medicore as a whole. Trust me, in games that have stealth as an option, you'll eventually resort to commiting great acts of violence in plain sight most of the time, anyway: Fable, Dark Messiah, Assassin's Creed, Deus Ex, Arx Fatalis, Morrowind, Oblivion...it goes on. :nono:
Thief is great because it is a stealth game, and only a stealth game. It doesn't do something like throw you in a big sandbox with some dark corners and clueless NPC's, give you a big sword, a magic wand, and a longbow and tell you to make a choice to choose your destiny, should that be your choosing (if that is what you choose).
But if atmosphere is what you truly seek, I recommend trying out some horror games. Not just FEAR or Doom 3, but System Shock 2, Silent Hill, etc. If you're in it for the stealth...just stick with Thief and its fan missions. Thief fan missions come out frequently anyway, so you'll always be occupied.

By the way, since syndef mentioned it: Is there anything else out there (game/book/movie/whatever) with this medieval-steampunk-crossover?

If sneaking is enough for you and you do not absolutely need that medieval world, you can try Hitman 2: Silent Assassin, Hitman: Contracts and Hitman: Blood Money. Although these are games where you play a contract killer, it is usually possible (and even rewarding!) to play levels as stealthily as possible, killing only the victim and using disguises and sneaking to bypass other people and obstacles. However, Hitman is set in the pesent world, with modern surroundings, pistols, machine guns, and so on.
If you're not sure, look for the demos. There are two for Hitman 2 and one each for the other two games. Each demo lets you play one of the game's levels. After that, you should know enough to decide whether you like that or not.
Beware the first game of the series, Hitman: Codename 47, since it is different from the other two, especially since you are not allowed to save your game (at least as far as I know). There's been a demo for it as well, so you could try that first.
Hitman: Blood Money has a strange way of dealing with saved games: As long as you do not leave the game, you may save and restore a game. But if you leave and re-enter the game, you have to restart the last mission from the beginning.

The Splinter Cell games are good for non-medieval 1st person sneakers. I'm a fan of Metal Gear Solid's stealth system too.
