As for the joke in the picture, this makes fun of the last strain of GUILT Savato. You will get the joke after you see this video on the game.
The joke is that this man made virus "Savato" looks and sounds like a virus made by Satan. The background music in the game makes it sound much more convincing! :wot:

Bully Oh no! You can get the game in America! Go to Gamestop. If you have a Wii, get Trauma Center: Second Opinion. If you have a Nintendo DS, get Trauma Center: Under the Knife. Both games are the same. The Wii version is just a remake with better graphics, gameplay, and music!

I used to have the DS game, I liked it at first but as soon as the ridiculously unrealistic elements started kicking in (special "virus" strains with colored attack patterns WAT), I got bored.
Still, it appeals to the main DS demographic* so that's all good.
*mid-to-late teenagers with interest in insipid anime and manga
