I tried to change the user.bnd file manually inserting mouse4 & mouse5 like the others but no joy.

Right, I seriously doubt there is support for those buttons in the 10-year-old engine.

Maybe remapping those buttons to a keyboard one with a external app. (i'm in ubuntu, playing on wine).

You could try using (
http://www.autohotkey.com/) AutoHotKey to remap the mouse buttons to another key. Somewhat awkward because you'd need to quit the script again when you wanted to use those buttons normally (e.g. for browser back/forwards).
AutoHotKey is customisable enough that you could probably write a script to only apply the mapping while Thief is running, but that's getting into heavy programming/scripting territory.

My Saitek works just fine.
Just used the profile editor that came with the mouse and mapped what keys I wanted.
Although all I use is run and clear inventory.

I have a Logitech and I just use the software that came with it to program the extra mouse buttons to press keyboard keys. It has a profile set up for each game I play so it only presses those keys when I'm playing Thief. Works great. So yes it is possible but it depends on your mouse and what software you have to work with.
