No love for Hidden & Dangerous 2? It's a great tactical squad-based war shooter which ticks all the checkboxes in the OP's wishlist. I don't think anyone who's played it will disagree with me when I say that it has some absolutely terrific mission design.
The one barrier to entry could be the controls, which can initially seem quite complex, but that sorts out easily enough after the tutorial.

The "tactical" side to Rogue Spear was bloody impenetrable to me. I must admit I really dug Raven Shield, though I will admit it's rather light on the tactical side but with plenty of good close quarters combat.

Seconded. Great game and the first game and its expansion are available as a free download called Hidden and Dangerous Deluxe. Google it to find it, it's one of the best bargains in the business.

I'm sure it's already been recommended (although CCCToad UNrecommended it), but GRAW 1 and 2 on PC were great games.
The second one is MUCH easier, but holy shit the first one will really test your frustration levels. Still, both fantastic games, although the second one has a lot more polish.

H&D2 is on my yet-to-play-properly list (with the expansion pack on my get-a-co-op-group-together list), so I'm unqualified to comment on that one, but it clearly fits the bill.

I would play h&d2 singleplayer first if i were you as the coop for it is slightly different from the normal game, altered in a more "traditional" multiplayer sense, with respawning and such. Sadly it does not work like the coop in the first game. Also, only the sabre squadron levels are coop - although i think others were being converted using the editor for mafia haha.
Incidentally, swat 3 on hardest at the bloody tv station level is doing my head in!
