On the PC version that was a symptom I had of the patch having not been applied properly (on 2k/xp, usually followed by a crash as soon as something died).

Bethesda says they'll "reinvent" the game series to make Fallout 3 an "open-ended RPG". Um, the original games WERE open-ended, Bethesda.
