Games that have successfully unhinged my nerves:
System Shock 2
Thief Series (including some fan missions)
Penumbra: Black Plague
Amnesia (most unnerving game I've played)
Haunted House (couldn't sleep in the bottom bunkbed for months out of fear that monsters would grab me after playing this game as a kid)
Scratches (though I dislike it)
Clock Tower gets honorable mention for being the best video game adaptation of the slasher genre I'm aware of.

Most of the ones I'd suggest have already been listed, but I got a few:
Condemned: Criminal Origins (... I am ashamed to say that a combination of fear and bad programming has prevented ever getting past the second level in this game. I hate getting attacked from behind, even after I thought I cleared a room entirely.)
Still Life
Bioshock- at least the beginning
Ravenholme- Half Life 2
I know Thief was mentioned, but damn The Asylum/Orphanage in 3 still scares the crap out of me. That thing should've stayed dead when I bombed it!!
Gotta love those psychological thriller games. Need more of those.

Yeah, that stage deserves to be mentioned specifically. Shalebridge Cradle is the only gaming experience I've had that lead me to consider shutting the game off, because it was just so intense. I pressed on, but holy crap was that nerve-wrecking to play through.

I think Condemned and CoC are my all-time favorite scary games, but since they were mentioned I'll just mention the first two Fatal Frame games and FEAR.

Excellent series. Classic Sierra style gameplay with a Lovecraft-esque story equals a happy gamer.

Bought Kuon for PS2...anyone know anything insightful about it? It was one of the few scary games on the system I saw that I had no experience with.
