As Beleg Cúthalion says, the saveindex.ion file is a reference file that indicates what saves are available, Each save game folder has a globally unique ID (GUID) which means no other PC will have the same folder (as its generated based on the time, and your PCs Mac ID).
The user options folder is generated when you save your config (e.g. change the key mappings in game). Any of the other guid based folders are created under the savegames folder every time you save the game as well.
I have a vista laptop as well that I have tried T3 on but have discovered that the best setup is to install it to "c:\games\thief -deadly shadows" rather than "c:\program files\thief -deadly shadows" due to vistas reaaaalllly annoying application virtualization.

A: When the game came out Vista was not available for the game to be written for.
B: The FAQ says always list your specs. There are thousands of different systems out there, both off the shelf (Dell, Toshiba, HP, etc.) homebuilt, and in laptops (for which the game clearly says no support is available, and the game was not written for), numerous operating systems (W98, W2K, XP, Vista), and a number of graphic cards that don't support the game, or cards that won't work with the game with the newest drivers. Many Gcards need to use older drivers, or need tweaks to make the game run. Nvidia and Gforce cards in particular have been giving the game problems.
