the funny thing is, metal gear: snake eater did it, sort of, by putting some black smudge along the right side of the screen when you're in first person mode.
but that's the only game i know of with such an effect.

Backwards mode: Always move backwards. You may never face an AI unless you are moving away from it.
Don't touch the ground: You may only step on objects (or AI) that can be carried. Blackjack people and use them as stepping stones. Moss arrows cannot be carried after they've been shot, so they count as ground.
Medium alert mode: Keep the AIs on medium alert constantly (never low nor high). A dead or unconscious AI counts as not being medium alert.
Ghosting from another point of view: Alter dark.gam and make every AI fully transparent ( = invisible). Then make them silent too.
No ears, no eyes: Turn off your screen and your speakers.

The ghosting from another POV sounds real scary for undead levels (their all ghosts!) but the no eyes no ears mode is just downright impossible, unless you memorize every single step in whatever mission your doing. Also, with no input, you could fall and die and not know it and keep playing for like 8 hours and wonder what the hell is going wrong!
