I have decided to choose Realistic in this game, is it the hardest level? What makes it realistic? What are the differences between the levels? Is it just primarily damage? Or does it include AI efficiency?

Mainly damage if memory serves. Its like, one shot to the head kills, which at last check was realistic.

Realistic is supposedly the hardest, but it's hardly hard at all if you ask me.

As far as I can tell, it applies strictly to weapon damage. It takes far fewer hits to kill you (and enemy snipers will kill you outright if they hit you).

Yeah I have definitely noticed the proficiency of snipers - ouch they hurt - tranq dart needs to hit the mark and I need to duck following a successful hit - dem bullets hurt.

Basically "realistic" gives you similar (or maybe even the same) damage resistance attributes as the ordinary humans in the game have. Headshots from standard pistols or sniper rifles kill you, and sniper rifles will take off a limb if they hit one. Getting fired upon by bots or MJ12 commandos is often terminal as well. Defensive augs become very valuable; Ballistic Protection can stop those one-shot kills, and high-level Regeneration can let you absorb fire from a LOT of guys with assault rifles... :)
Combat suddenly hurts a lot more, but the AI doesn't become any more prevalent or proficient. They're still just as easy to confuse and get away from.

Just squat during combat and you won't have a problem (with human opponents).
I wish they would include a harder difficulty level with just about every game I've played. It sure would be a major replay value...
