I do think it's a shame there's not a "Proper" ghost achievement for this game, where even KOs count against it. Of the levels I've played so far, I suspect it's entirely possible to completely ghost them, especially with clever use of Blink.
A favourite of mine is to lean around a corner, see a guard coming toward me, blink behind him and KO. I'm sure that could be used for ghosting too.

No they don't count. But if you knocked out any guards, and they either fell into water or were found by rats then that counts as a kill.
One side objective I'm curious about is
the duel outside the masquerade party. I never even considered using sleep darts or something, I wonder if the game counts that as a kill when it basically forces you into a surprise gunfight.

Sokolov of course, I think it must have counted the 5 people from the Bottle Street gang who were killed by the City Watch in my play-through of that level, in which case that's some BS.
2 were killed the front of the bridge by the gate and 3 were shot dead by the watch after I
was jumped by the guy who I helped escape and his friends, slowed time, blinked out, and the watch spotted them.As for the
Duel I believe it does count. I froze time, shot a sleep dart, and fired the pistol above his head. They still thought he died, it was great.

About the aforementioned side-quest: [spoiler]I dueled him so that I slowed time down and shot him with a sleeping dart. I did not fake the gun like Scots did. I thought I was really brilliant, but the game let me down quite badly there: the guards checked his pulse, said he is dead. Only problem is that the guy snored like hell. This feels like they didn't consider players doing this. However, even if this is badly designed, it works, so you can do it without killing him. [/spoiler]

Having finished with general mayhem on every level I decided to start again and see how close to ghosting I can get. But first mission seems near-impossible.
SPOILER: MISSION 1 ONLY:
The forced explosion and subsequent dive into water watched by guards makes me wonder how one can ghost this. The only way I can think of, with hindsight, is to KO all the guards in the prison before blowing the door which is not really ghosting. One other possibility might be after using the lever on the door before to then frob it again and dash through the door before it closes - all without being spotted. Then KO the one guard who is permanently staring forward. Or is it possible to distract him with a thrown bottle? But to where? It's not like you can send him down a side corridor out of the way. I would regard that section as very difficult to ghost even though I'm playing on Easy! (Xbox)
Still a very good game though - though only nine levels. Makes our Dark Mod campaign seem more substantial because I'm sure there'll be more playtime to be had completing that when done - and for free! Apart from TDM, Dishonored is the nearest game I've seen to Thief-style stealth.

I've read that some people have run through just after the explosion without alerting the guards so I'm thinking that it might just be a matter of timing. Alternatively I'm wondering if it might involve hiding in the trashcan to the right of the door just after setting the device and then waiting for everything to die down.
I'm stuck on the very first set of guards in the first mission (one facing the whole room, one who turns 180° and walks towards a window and back, and one who patrols back and forth). I can't figure out how to pickpocket the guard who turns 180° without alerting the one facing the whole room. He's near a caged window, but he stops his turning routine once you get access to the other side of it, and I'm not sure it would be possible to pickpocket through the cage anyway. Has anyone managed to ghost (without KO) and get 100% loot in the first mission? I think I've managed to get the rest of the loot, but he's got me stumped.

I took more care replaying the first mission yet still only found about a third of the loot!

Mind you, I skirted the guard camp in the sewer. what you said about waiting for things to die down might be the answer. I never thought of it 'cos its kinda crazy to set off an explosion and that's not a major alert. But I guess if you're not seen then you ghosted. Whether they won't notice you in that trash thing I don't know. That first guard I couldn't even get by without KOing him let alone pickpocket the 2nd guard.

As for the explosion - just blow the door then hide in the nearby dumpster until the guards wander off. Then sneak into the sewers and no one will notice.

I'm doing Shadow/Clean Hands and so far I only absolutely had to knock out one guard, the first one in the game in the prison. I found no way to get past him without KO-ing him, which was frustrating. Also, I've found that some stuff required you to KO or damage property to get to them. I grabbed them but if I want to do a stern and old-fashioned Ghost playthrough, I'd have to leave these be. I do believe most of the missions can be ghosted à la Thief, though.
Oh and yeah, I had to restart the Coldridge mission twice before I found out how to get past the two guards and the explosion undetected. :p

Doesn't really feel like ghosting though (blasting open the door to the prison with explosives!)
Now another side objective that seems undoable to ghost: The guy called Gruff or something boarded up in the old house by the bottle street gang thugs. I tried sleep darts but could not do it without the other guard alerting. I tried sneaking by possessing a rat but then KOs were disabled! Why? I tried as a rat again meaning to hack down the door behind them and run in and hide but I never could get behind them again with an alert. I tried a diversion - I threw a glass down the street. They grumbled but ignored it. I shot an incendiary bolt down the street and they just said 'who's making all the noise?' but didn't move. So I'll leave that one I think.
