You get that key from one of the female servants after kidnapping Sokolov. She'll mention she uses it as a hideout spot, and give you a key.

How do you drop Grannys Cameo into the Furnace? It's in my inventory, I can open the door, but I can't access the item.

Does Granny seeing you in that fight count against ghost? I have to redo that level since I blew it somehow, and I'm not sure if it was her, or if I blew it somewhere else - that's a long ass level.

I saw a page that had the achievement requirements listed.
it said ghost = not being seen or alerting anyone EXCEPT major plotline characters.
So as far as the achievement goes you should be able to fight her and still pass.

Except of course that she isn't a major plotline character, so she wouldn't count for that exception.
Phatose, as to your spoiler specifically:
There are a couple ways of dealing with that without getting an alert. One is to pickpocket the key and just sprint back out of the room the way you came in. Another, especially if you have the time stop ability, is to stop time, pick pocket her, then sprint up to her 'hut' and get into the back room immediately, stopping time as necessary to make it. She doesn't actually 'alert' until she sees you after giving her speech and teleporting up to her hut, causing the 'combat' to begin. So as long as you don't let her see you after she ports up there, you don't lose the no alert condition.
Another interesting thing is that, if you don't finish all of her or Slackjaw's sidequests, that entire event doesn't even happen. Very weird, but useful.

Is there any way to break out of the prison without alerting the two guards (or more) when the bomb explodes? Also think I get one killed... somehow. Would like to avoid it. Also is there a way out of the prison without using the bomb? Now that would be interesting.

There doesn't seem to be another way, no. Have you put an unconscious guard in the dumpster near the explodey door? That might've killed him.

No, I pretty much went all the way without them even seeing me, then waited till the one nearest to the door was far away next to the console that has the switch which opens/closes the big door. For some reason I had a kill counted at the of the sewers (even though I had not even knocked out anyone) so I assumed that one guard died in the explosion. *sigh* I don't know, I've yet to understand how this game counts kills, especially with one bug it has with unconscious npcs.
