Ok, I set the switches to the original positions.
[SPOILER]I then figured that I needed to move the levers in this order: 642135. The ending positions are up up down up down up. I hit the red button and walk over to the on/off switch... [/SPOILER]
It isn't selectable and the gas is pouring out.
I am going insane with this last puzzle... so what am I doing wrong?

this puzzle can be a pain for a lot of people, if you screw up the puzzle you have to backtrack it in the exact order you started with to reset it. the problem is that the way the order specific puzzle is set up there isn't a reset, so if you make a mistake and then go on you have to slip the switches in the same order back to the mistake then proceed again, this is tough. in version 1 the gas in the chamber can be avoided if you edge up and lean in it to grab garrett, the radius was increased in version 2, this assumes that he fell forward when he died, he's alive at the start of the mission so that his body position can change.

if you have a hard (not quick) save before the puzzle give it another go flipping the switches.
26) How do I disable the Noxious Fumes?
A) This is an order dependent problem, in the first row are 1) the outdoor vent, 2) the tank outlet valve, 3) the tank mixer valve. In the second 4) the chamber inlet valve, 5) the master bypass valve, 6)the gas shunt valve. The order is 1) gas shunt valve, 2) chamber inlet valve, 3) tank outlet valve, 4) outdoor vent,5) tank mixer, 6) master bypass valve. Looking head on at the panel the switches are thus numbered, 1) first row: 435, 2) second row: 261. Flip the switches in that order, press the bottom red button and the on/off switch becomes active.

I'm in the dungeon with the pagans. There's a steel door in the dungeon. How do I open it? I think I picked it with one pick, but it won't let me pick with the other and it won't open. Looks like it goes down, but it may just be a cell.
If that's not where Garrett is, where is he?
EDIT: Found the solution
[SPOILER]I needed the third lock pick which is upstairs on a sill in the empty cell near the drunk. [/SPOILER]

[SPOILER]the door on the north most part of the dungeon is the right one, if you are on expert its pickable, though requires the third lockpick. on lower levels a key is in the offices in the dungeon on the desk.
you can not advance far in this mission without finding the third lockpick in the holding cells.[/SPOILER]
edit: i meant north, not south, i forget sometimes when i haven't been inside it in a while.

I had a problem with the escape objective.
[SPOILER]I left through the sewers with garrett before I got the antidote. Put him at the bottom of one of the ladders that leads into the sewers from the outside. Went back in to make the antidote and then I escaped through the other sewer ladder. When I went to pick up Garrett, even though everything but escape and don't kill were unchecked and even when I went down, picked up garrett and took him to the starting point it didn't check off. I had to carry him back to a sewer gallery near the electrical room entrance to the watch. That's when both remaining objectives were checked off.[/SPOILER]

the exit points are the tunnel you entered through and the top half of the large room there, it appears that if you have a missing objective and go back for garrett it has some trouble checking it off, it should be easy to back track around and get it, your not the first to go all the way to the start and have nothing happen.
