The music in
Ultima VII (both Black Gate and Serpent Isle) is simply amazing. How can some (
http://www.moongates.com/Music.asp?midi=U7-Stones) simple MIDI tunes be so great and memorable? I must have played that game for hundreds of hours but I never got bored of the music.

Another game that has a brilliant soundtrack is
Outlaws (the old western 3D shooter by Lucasarts). The music by Clint Bajakian is very much inspired by Ennio Morricone, which is a good thing obviously. The game itself may be dated and ugly, but I still play it every now and then, just to listen to the soundtrack.
Also, pretty much everything that Richard Joseph (R.I.P) ever composed is quite awesome, for example the theme music in
Sensible Soccer, (http://www.youtube.com/watch?v=PiYuq6Ac3a0) Cannon Fodder, Speedball 2...
I loved the music in
Wing Commander, I don't know who has composed those songs though. The (

http://download.wcnews.com/music/wc1/wing%20commander%20-%20death%20and%20funeral.mp3) funeral song ( :( ) makes me want to go blow up some Kilrathi furballs again.

Zelda: OOT has some excellent music.
AvP had some good atmosphere music if I remember correctly. At least that was until my cousin put a huge scratch across the CD.
Finally I used to love the theme tune from Worms.

Reznor, yeah. Still my favorite thing he's ever done, at least until Ghosts came out.
On the subject of least favorite game music, I hate pretty much every soundtrack that is either completely generic Holst-ambient, Williams-style quasi-romantic, crappy electro metal, or trance. I wish more people would go the Quake/Thief route, or do stuff like the soundtrack to Pi.
Oh yeah, I forgot a favorite - Indigo Prophecy. Angelo Badalamenti doing game music = spoo.
Edit: Koji Kondo is pretty much universally awesome. Also, the main theme to Space Channel 5 is great (Mexican Flyer by Ken Woodman). Both Jet Set Radio games also had great soundtracks, particularly the tracks by Guitar Vader and Hideki Naganuma.

I see most of my favorite game music has already been listed. I wanted to add the (
http://de.youtube.com/watch?v=N5uC_my4Vy4) Max Payne 2 theme. So sad and foreboding.
Also
Gothic 3 soundtrack is fine piece of classic music.
And I haven´t seen a mention of
Call of Juarez soundtrack, which is great. The soundtrack, not the fact that it´s not been mentioned. Especially that one theme that gets repeated throughout the game and finally comes with vocals in
Chapter 10 (me thinks), where Reverend Ray after finally meeting Billy, enters the mine to take all bandits out. Fucking epic.

As far as good soundtracks go, a few games spring to mind - Drakan, Unreal Tournament , Deus Ex, and the title theme of Soul Reaver 2. I can't shake the feeling that I'm not doing justice to a couple of other games I played that had awesome music, but screw it, I'm tired.

Never played Felghana, but I was gonna say Ys I & II, and Ys III [for Turbo Grafx] have easily the best game music ever, and WAY ahead of their time, too.

Anachronox has an amazing soundtrack. There's so freaking many tracks, and there's so much variety, it's almost certain there's going to be something in there for everyone to love. Conveniently (and unusual for a game this old), all the music is stored in MP3 format.
VtM: Bloodlines has a great mix of ambient tracks and straight-up club music, but unfortunately there's not very much of it.
