Hm, it's kinda hard to choose the number one, but I guess I'll work my way up to it...
First of all, it's a bit from a Half-Life mod, so I don't know if it counts, but Somos from halfquake amen is a very weird bit of 'music', but I always liked it. It's played when you get though the extremely weird levels when you face off against the final boss Somos. Not sure why I like it, but I tend to add it to any music list I make. :)
Next up, I would say the main theme from Duke 3D, even if the game itself doesn't interest me, the main theme is one I can never forget. It just feels like a 'kicking ass' kind of song.
Um, hmm, title theme for Buried in time (Journyman project 2). It just sounds so EPIC, makes me happy that they used some of it in the begining of JP3. :D
I guess this would be second place, but Club Doom from one of the playstation dooms. As I remember the music doesn't kick in till you enter the 'club' where you get to fight in a dance room with colored tiles and light blinking on and off with Revenants dancing on the dance floor. Something your don't forget when you find it. ;)
But I'd have to say that the music I can never seem to get enough of, which would make it my number one fav, is Stage 1 of Blastermaster. It's a tough game to beat (without cheating that is. :o), but the music in the first part of the game just gets me going every time.
Some music that didn't make it was You're not here (Silent Hill 3), The Dream will Never Die (from the SNES version of Tales of Phantasia), main music from Faxanadu, Simple and Clean (Kingdom Hearts), and finally the stage music for Infinity Mijinion (Mega Man X, forget the number). Thats just what I can remember off hand. :)

I love love love <3 Ico's main theme and end theme, and the tune that plays on Planscape:Torment's title screen is really good too (been so long I can't remember the rest). And I think Adam Freeland's track (Fear, I think?) for Rez is superb - the game mix is far better than the soundtrack version (that goes for the whole soundtrack IMO).
The 'puttin' out a fire' and mad pilot songs from Um Jammer Lammy are fucking awesome. And everything in Vib Ribbon is sup-erb*, but the first track (Polaroid?) is absolutely amazing. Katamari goes without saying... I think my favourite is Angel Gifts (is that the right name?), even though it's quite different to the rest of the soundtrack. And another vote for the whole Outcast soundtrack here, my favourite being the city theme.
* that's a typo, but I've decided to keep it.
If you're willing to use a bit of imagination, the SID-chip achievements of Beyond the Forbidden Forest and The Last Ninja (and yeah, I know, countless other C64 games) are quite remarkable.

My favorite single song is the main song from Devil's Crush (a pinball game for the turbo grafx 16).
...or maybe any song from Ys Books I & II or Ys III (also for turbo grafx 16... NOT the stupid nintendo version).

I'll admit that normally I don't pay much attention to the music. It's nice to have a good background score but I don't normally pay conscious attention to it.
Having said that, the only piece of music in a game to get any real emotion from me was the sad tune from Metal Gear Solid. Enclosure, I think it was called. It really added to the death scenes where it was played, especially at the end of the "bad ending".
A lot of tunes from the SNES Final Fantasy games I played also were catchy/good.

Ocarina of time was the best hands down. Evocative, yet simple haunting melodies, mostly based on modal harmony, creating an open, ethereal like quality to the tunes. Also, you got to play the main melodies in game on the ocarina!

Jets & Guns is a great shooter and also has great music made by a hard metal swedish band.

I always found the Ocarina of Time music to be not quite as good as the other Zelda games. Sure, there are songs I enjoy like Gerudo Valley, Kakariko Village (that's from LTTP though), and Kokiri Forest, but much of it just doesn't hit me as strongly as it does others. The Ocarina songs are great, but some of the area tunes...I don't know. I believe Link to the Past has far better music, as does Wind Waker and Majora's Mask.
On another note, does nobody think anything of the Earthbound music? It might just have the best soundtrack of any game.
