To my mind no game has ever had a soundtrack that is consistently brilliant, fun to listen to, and matches each distinct level perfectly as the Genesis era of Sonic the Hedgehog series (that means Sonic 1, 2, 3, Knuckles and 3D).
Many different composers, hundreds of individual songs -all classics. To name a few that stand out: Green Hill Zone, Labarynth Zone, Casino Night Zone, Hydrocity, Ice Cap, and I could go on forever.
Honorable mentions
Hall of Souls - Trent Reznor - Quake
Caminis Theme - Alexandor Brandon - Tyrian
Underworld Part 2 - "" - Unreal Tournament
You've got to Eat your Vegatables - Bobby Prince - Commander Keen
Duke Nukem 2 Sountrack - Bobby Prince
Raptor Title Screen - Bobby Prince
The entire Doom 1 and 2 Sountrack - Bobby Prince
African Mines/ Mountains - Unknown - Ducktales
Options Screen - I forget - Gradius 2
Bonus Stage - Unknown - Zanac
Clocktower - I forget - Castlevania 3
Level Selection - "" - Mario 64

Honestly, Star Ocean:Second Story is #1 for me. That game has an INCREDIBLE soundtrack. Words don't do it justice. Most anything from the Silent Hill games, Deus Ex (UNATCO and Hell's Kitchen especially)are others that stand out to me.

Most of the iMuse music for Monkey Island 2. I love the Dry Bones sequence.

Lost Painting by Michiru Yamane, which plays in Inverted Library (and others) in the game Castlevania: Symphony of the Night.

Koji Kondo does all of the Mario and Zelda music (except Zelda 2, if I recall correctly).

Ocarina had great music, for the most part. Some of it was kind of average. The opening sequence was very well done. From what I have seen, I really like the music in Wind Waker. Took some getting used to, but I'm very tempted to play through it now.
Speaking of Zelda, (

Yeah, Wind Waker music was good too. All the usual Zelda tunes sounded great, and you've got to love that main theme :cheeky:

Actually, I'm going to have to declare Anachronox' "Mandance" (and the associated "Stiletto Edit" track) simply because I can't believe that was written for and featured in a goddamned
game.
Seriously. Especially after the 2:30 mark onwards to the end.
I think it was a local band one of the developers knew - picked that up on the generally dead planet anachronox forums somewhere.
Surely the Democratus theme is also worthy? Bit difficult to choose with Anachronox. Pity they overcooked the mp3s - nasty artefacts on most of them.
There was one small piece from Neverwinter Nights I thought was almost perfect - the file is called mus_castle I think. Jeremy Soule is really rather good.
However, the best stuff I've heard recently is in Max Payne (yes, got to it late). The music that starts up when you approach the blown out safe in the subway station is just right for the situation, but you probably wouldn't listen to it in isolation much.
For a single favourite I'd have to go back to 1991 and choose the main theme for Turrican 2. Chris Huelsbeck managed to squeeze a lot out of 4 channels of 8-bit sound on an Amiga. It was actually extended to 7 with a bit of clever programming, which was probably why it sounded somewhat "fatter" than usual. This is probably partly nostalgia - I don't think many people here will have heard of the game. I'm not sure how many old Amiga users hang around here...
