The game gets points for a pretty up-front portrayal of necrophilia, though. Not sure a game has ever done that.

I'm surprised no one mentioned System Shock 2. Maybe I'm naive, but the first time I played it I never saw that one coming.

Hey, I mentioned System Shock 2.
Rule of Rose had a mindfuck edge and that kind of twist, and the cutscenes were really well done in messing with your head, but it gets pretty much universally slammed for the boring gameplay in the meantime.
Anchorhead has a pretty awesome plot development on this front, though it's interactive fiction.
Then there are the surreal type games, not horror, mostly indie, and if you're not in the mood they probably won't do anything for you, but if you are they can at least massage your brain a little. Let's see...
LSD, Yume Nikki, the La La Land series, some of CactusMilk's games: SpaceFuck, Psychosomnium ... All still pretty tame and pretty much gimmick games, though.
SchoolDays is a J-romance game (highschool at that) about a fucked up love-triangle where there's plenty of murderous jealousy go to around, though that's a different kind of mindfuck maybe.
