Since we've already way broadened our categories from horror ... another one to mention is Alpha Centauri. It's also another one with a literal mindfuck you can use as a weapon.

Well. Siren: Blood curse can screw with your head exceedingly well in places. I've heard its earlier PS2 brethren have similarly fucked-up narratives too.
Not just similarly, but more so, I would say absolutely fucked-up.

Been a long time since I played it, but there was an isometric game from the 90s called Sanitarium which freaked me out pretty good (at the time, at least).

If you have cube/wii/ps2 get Killer7. Now that game screws with your head, one of my all time favorites.

Yeah, Killer7 is one of the biggest head-fucks I've experienced in gaming. Completely bizarre, in a very David Lynch fashion.

Very timely! A title that always appears in lists of "really great games with god-awful voice acting", IIRC.

truly god-awful voice acting, but a very good game very disturbing, it's creepy because it's so strange nothing to hold onto, bit like what goes through your head when you can't sleep. Well chez Fragony at least. Killer7 has that same quality by the way, that detachment from reality it makes you feel really strange, mindfuck isn't really strong enough this is rape.

Would you recommend it, though? I feel more or less the same way about David Lynch's
Eraserhead, yet I would say it's a worthwhile movie to watch, if you're into surreal, disturbing imagery.

Well Killer7 doesn't have much to offer in the way of gameplay. It's basically an on-rail shooter, except that you have to move forward manually. And it doesn't have any of the set-pieces one might usually expect of an on-rail shooter (the enemies rarely do anything more exciting than run straight at you). Personally I started it but gave up after a few missions. It certainly is a mindfuck though and I'm a big fan of the artstyle. But unless you can find it for 5-10 bucks you might be just as well off watching the gameplay videos/cutscenes on YouTube.

I absolutely loved it, so yes I recommend it, but you might end up with something you hate, try it. It's a unique game there is nothing like it, I would buy it just because it's a peculiarity that belongs in a gamers collection.
