Oddworld: Abe's Oddysee. Loved that game when I was a kid (still love it), but holy crap some areas in this game are a real nightmare, especially the secret areas. In comparison Exoddus is really easy and forgiving.

The game I really wanted to love but sucked too hard to make any progress was Operation Flashpoint.

I don't know what Rockstar were thinking when they designed the games' flying mechanics.

There's nothing wrong with the flying mechanics, dude. Perfectly straightforward arcadey flightmechanics. It's not like you have to worry about flaps or windspeed or anything like that.
Unless you're talking about the awkward keyboard controls for the helicopters, then yeah, those weren't great.

Playing GTA on a mouse and keyboard is the definition of insanity anyway.

There was a problem with GTA: SA and vsync. I can't remember whether it was when it was off or on, but one of the settings screwed quite badly with controls. The mission where you have to swim/dive through rings, for instance, was unplayable. I believe that some driving/flying controls were similarly affected. I remember the donut driving lession being impossible with vsync switched... on? off? Can't remember.
In any case, there were certain missions that were almost unplayable in one state - and then, after I changed the settings, they were remarkably simple.

Pretty old school, but I never did manage to finish R-Type.

There are so, so many shoot-em ups that I never finished. I don't think I finished Ikari Warriors, or Green Beret. Or Commando come to think of it. Never finished Turrican or Turrican 2 either I don't think.
I definitely didn't finish Knight Lore though I may have finished Paradroid.

The GTA flying mechanics are so hard compared with e.g. Just Cause 2 or Prototype. Every other mechanic in the game is simple to perform and those which aren't (e.g. some of the driving school stuff) aren't mandatory to progress. If the 2 aforementioned games can let me fly flawlessly on kb + m, then I'm going to go ahead and call GTA's flying out on that.
A quick look through google tells me I'm far from alone in this.

Lots of old school games were very tough to complete fully. I loved exploring in the environments in games like Spindizzy and Jet Set Willy even if I didn't have the skill or patience to complete them.
As a more modern example Super Meat Boy defeated me with the last levels. Loved the game to bits but eventually it became more frustrating than it was worth.
