It's a stupid continuity error, the eye thing. And the scar... well, that's just a stupid concession to a stupid focus group full of stupid people and then okayed by other stupid people in charge of making stupid decisions.
But it's okay now. They've mostly been fired.

I'm thinking the TDS scar was designed to draw attention to the fact that there's something odd about his eye, for those who wouldn't know Garrett's story.

The scar never did make much sense to me. Viktoria did a pretty good job about not causing too much extra damage. Maybe she does night-shifts at the local hospital?

Thankfully it was the proper eye in the actual game and cinemas.
Still doesn't excuse the scar though. I guess they thought it would make Garrett look XTREME!!!!!!!! Sometimes I'm surprised Deadly Shadows didn't have dual wielding.

On the topic of the eye:
I've just completed TDP (for the first time) and i've started playing TMA (with the intention of finally completing it) and my question was (for me) the eye just seems to appear in between these two games with no explanation.

An explanation comes up later on in TMA: Karras, while he was still a Hammerite, made it for Garrett.

The mech eye can be seen in the end cutscene of T1. So it doesn't appear in between the two games but at the end of T1.
