I usually play my games on Easy or Medium nowadays. Or whatever they call their equivalents of those difficulty concepts. Your post, in its current form, seems to put me in your hate list. The fact is that I've lost my interest in pushing myself towards higher degrees of difficulty quite some time ago. Yeah, I used to constantly try to challenge myself with the higher AI levels in UT, for instance... back when I was 15 or so. I'm no longer interested in calculating builds, or honing my reflexes with a mouse and keyboard, and the thought of having to attempt an encounter for a dozen times, because I've got other activities than gaming that also need attention, be they pleasant or not, isn't very appealing - the only thing that may drive me these days towards higher difficulties is the impact it may have on the general experience, the way it can factor into my immersion. My primary interest is in concepts such as choice & consequence, "player driven narratives" (dead horse, I know, but hey), or simply experiencing, if I may say so, worlds and events/actions that just can't be seen by walking out your front door, or turning on the TV. To exemplify - my preference for games like KOTOR or Deus Ex, rather than stuff like GTA or Medal of Honor.
Not everyone who doesn't play on Hard fits in the bucket you're spitting at. There are people who simply value a different aspect of this medium more than the competitive component of a game, and most of us don't give a flying fuck about high-difficulty bragging rights. So, for my part, I'd kindly ask you to have your rant without shitting in my general direction, 'cause you're barking up the wrong tree. The fact is that your post would've probably been much more to the point if you'd centered it around your ending sentence, rather than your current title.

You can't be serious. You're expecting too much from likes of EA and Ubisoft. If you've seen how badly optimised MoH:A and R6 Vegas 1+2 are then you'd know that the last thing that they would've touched are the difficulty settings. Infinity Ward is the only developer from those examples that may've done so.

To my mind there are two separate issues here:
1) Difficulty levels within games.
2) Mastering techniques in a game.
I think that there's a case for 'difficulty levels' to be removed from all games. They're a purely artificial construction where on the whole player or enemy hitpoints, damage dealt or skill are capped in an artificial manner. They're convenient, but redirect the challenge from required skill to playing with differently weighted dice.
The second aspect, mastering game techniques, was one that was required in a lot of older games because there wasn't much alternative. Repetative gamplay was compensated for by encouraging the player to become better at the same situation while exposed to gradually increasing danger. The old arcade games are a perfect example of this - your survival depended on your grasp of the game mechanics but once mastered your effort was rewarded by being able to play for a long time. These days arcade games charge for the time you're using them and skill has little to do with it.
Overall, I think the best solution is for games to be adaptive to the player's skill. It's tricky, requires some sensible design choices but ultimately is the best way for all skills to be catered for. It doesn't have to be as artificial as increasing enemy hitpoints or their ability to shoot from the other side of the map.
Just allow different options for tackling a game and that's a form of difficulty modifier in itself.

Well, devs who are not fucking lazy twats usually put more effort into it, even if it's just as simple as spawning more/better enemies at you. See Q2.

Most of what needed to be said was reflected in my peers(with less of a chip on their shoulders I suppose). The rest is up to speculation and personal preference. Which I really do like getting insight on. Tear me apart.
