Regen health/spawning enemies if used correctly are fine. And yes, I was brought up on backtracking for medkits through half a level. And thats fine too, in some games.
In Halo the shields are perfect. Logical/well explained, too. Aliens have them, so that`s sorta fair. When they removed medkits in sequels I was initially pissed off, but then adapted...in this kind of shooter - more frantic - shields are more important. Also i did all my backtracking for ammo/grenades already.
Crysis ? Finished it not so long ago and I didn`t really observe such nasty cases of alert spawns as CCCToad says. Not saying they didn`t happen but I was actually too busy playing that game to ponder on design decisions. Because it was incredibly good is why and also it was quite open - so can you really track the whole area in your head?

I had a pretty different experiences. Once I got past the first level, I ended up kiling at least thirty enemies in just about every firefight I got into. Quite often these guys popped out of areas that I knew were already clear. In once case, I had finished clearing a building and triggered an alert while firing out the hallway. Lo and Behold, about ten guys came running down the stairs. If I clear a room, its fucking clear. I highly doubt that an entire squad was able to hide up there without me knowing.
Basically, every level of Crysis was like the Library in my playthrough.

Not saying it didn`t happen - think I remember similar stupid "upstairs" situation, when 1st tank appeared.
Two things though: games you compare Crysis to have way smaller levels - or like SS2/Bioshock are totally locked in space/water. Hence less enemies. In Crysis you deal with a blasted army in a jungle - think it`s fair enough to have few squads deployed. Also you`ve got that suit - serious advantage. So playing on hardest difficulty I`ve found it just about right.
Second: I don`t find many games that I could call awesome these days, but when I do, I excuse them for some slight flaws...especially when it comes to more open design.

Bollocks. I was never wavemobbed in Crysis, and I've played it on Delta twice over. The maximum I've ever had to deal with at a time was 5-6 even after alerts apart from a set-piece or two: these guys usually travel in packs that are easily taken out, so you've got to be singularly inept or running around and around like a headless chicken attracting the entire level's worth of attention if you get an entire battalion on your ass.
It never even comes close to CoD levels of trash mobs let alone motherfucking Starcraft, so I have no idea what you're trying to prove with your hyperbole.

I'm with Toad. Having enemies that just keep respawning stinks. Fragging becomes a chore while I look for objectives to complete and it makes the game feel completely unrealistic.
The only game to pull these things off well was GTA... and my health didn't respawn there.

Seems we had pretty different experiences. In my playthrough, I'd count at least twenty red dots on my sensor in the majority of encounters. After I'd wipe them off the map, I'd usually turn and be mobbed by another dozen or two. They swarmed like teenagers in a mall on Saturday afternoon.

Who cares? He's obviously incredibly inept at playing Crysis and probably zoomed through every camp he came across in a level like a streaker during Super Bowl before turning around and dying.
Either that, or he's confused his last playthrough of Crysis with the first time he booted up Sexy Adventures on Porn Island.

IIRC, Crysis doesn't spawn enemies when you're detected, but if they get off an alert flare, the reinforcements come from hammerspace. Spawn out of visual range and have to drive to your location, but I'm fairly certain they're not already on the map.
