It probably got AO due to the context of the killing spree along with many of the targets being civillians (who apparently beg for their lives).

ERSB ratings are self-reported. You basically fill out a questionnaire about how violent/pornographic/etc your game is. The devs probably exaggerated a bit on the report to get a higher rating.

Not quite. More like a pretty short (4-5 hours), buggy, and poorly optimised sub-par twin stick shooter with some nice visuals and building destruction.
