The designers probably just wanted the spiders to surprise the player by having weird field of vision.

If they're visually "close" to a spider then yes. The closer you get to the classic spider shape, the worse it is. Thief spiders aren't too realistic, but they trigger the phobia very efficiently anyway.
Let me also say this: fear and phobia are two totally different things. Fear is rational, while phobia is irrational. I don't fear spiders, I don't even dislike them. I think the arachnids are a wonderful and incredibly diverse old branch of evolution. The "concept" has been around for something like 400 million years and currently there are tens of thousands of different spider species that we know, and many many more we don't know. Well, suffice to say, I would actually like spiders, if it weren't for some obsolete hardwiring in my brain.
