Ren & Stimpy was gross because JohnK knew kids would think it was funny. Binding Of Isaac is gross because...well, I have no idea really. It doesn't seem to add anything to the entertainment value.

The game mechanics would work/have worked in a more traditional fantasy setting. For example, being able to make a deal with the devil is an extremely dilute version of praying to nethack/crawl gods. The little death note at the end is your hack bones file or whatever.
However, without the absurdity of some of the power-ups, we would be retreading old ground. Shoop da whoop (MAGIC WAND!!11) or the onions (+1 arrows! YAY THAT TROPE ISN'T AT ALL OLD) being examples. It is gross, yeah - you're working your way through some kid's crazy-ass nightmare world. I guess it could be less gross or less POSTMODERN AND SURREALISM, but making another Dungeons of Dredmor would just be another... Dungeons of Dredmor.

TBOI is growing on me. I think some of its failings will bug me permanently, but I certainly appreciate the variety of the game now. I've managed to unlock Eve and Cain, but I don't think I've gotten more than four levels into the game so far.
I also think the cat's name should be randomly selected from a giant list.

I love how TBOI mixes grotesque-gore-lololinternetmeme style with some actually disturbing traces of... I don't know, real backstory? The two powerups that add up to your firepower, Brother Bobby and Sister Maggy, are (aborted?) fetuses, and there are various items in the theme of child abuse - the Wooden Spoon, for example, adds spoon-shaped bruises to Isaac's face and permanently increases speed - if somebody kept smacking me, I'd learn to avoid it quickly.
I like how it keeps that sort of stuff in the background and doesn't shove it down my throat, like many "art games" would.
