You may be misunderstanding the application of the rule. If one does get overly worked up or goes off the rails as a drama queen, naturally they have to be brought back to Internet reality of chill out, or the rule doesn't mean anything. So one can care about people not flipping out--this is not inconsistent with the rule--if the ultimate goal is to actually fulfill its deeper purpose of everybody chill in the end.
A further corollary I should mention is that you can get worked up about things that happen in the real world of course, if they are sufficiently horrible, involve you, and you're flipping out in the real world and it's not just a proxy for what's in really at its core just Internet butthurt, to use the technical term. It's just Internet matters that are best shrugged off with chill for everything to operate more smoothly.
Edit: I should also say this is just my own approach I've found works best in my experience. Naturally other people approach Internet differently, and there is room for plurality, or what's a worldwide web for. But I'd still recommend chill to everybody in most cases.
