I don't think the gameworld needs to be any bigger. JC2 was big as hell and most of it looked like it had been procedurally generated without any real attention to detail. I'd rather have more actually thought-out locations(like the blimp, and the Lost-island).
Oh, and better car-handling of course!
And maybe the ability to use the grappling hook to swing with, à la Bionic Commando. Then again the grapple/pull/parachute system in JC 2 worked pretty well already, maybe adding swinging to it would just muddle things up. Don't know. I'm hoping the devs are experimenting with it at least and trying to wring more functionality out of the grappling hook tho.

the obvious addition would be some kind of co-op mp, if it could be pulled off. Imagine hooning around tropical islands with a couple of mates, doing some crazy stunts.

This. Much as I loved JC2, after blowing up the umpteenth generic oil refinery/base/town my attention began to wane.

Also we definitely need to be able to hook massive oil tanks to Chinooks, which we can then drop onto supertankers which we've hijacked and parked in enemy naval bases. And being able to blow up bridges at opportune moments would be nice too. Also, can we have trains please.

How about planes which don't point explode? If you skyjacked a 747 and crashed it into something, it would only actually damage the first thing it hit instead of causing a massive explosion with debris.

I would bet MP is top priority for them. Would make the game so much more alluring for new players.

Er....I was unclear. In JC2, if you crash a plane into a building, there's no big explosion. The plane blows up and only actually counts as a hit the building. I want there to be a huge explosion with pieces everywhere, so if I crash a plane into a convoy of trucks, they all go boom - instead of just the one the tip of my nose hit.

What I'd like to see is the game still walking a fine line between "take my missions seriously" and pure lunacy. Physics are just exaggerated enough, keep the silly and cliché bond plot but please, please don't tread the Saints Row path. I certainly don't want to be fighting Deadmau5-masked fluo enemies with dual fake vomit launchers.
World doesn't need to be any bigger, there was plenty enough to explore, at least on the outside. Add even more easter eggs, have some reward you with a mission or an item. Make the gunfights interesting and bend the difficulty progression curve into reasonable shape. Fix the method of obtaining/buying weapons and ammo. Fix the controls, of course. Stop making each and every story mission end up in a military base gunfight.
Trains would receive much love from me, as well as the better accident/explosion handling that Phatose is describing. Populate jungle with animals and more variety in settlements. Make the big cities alive (goddammit). Implement a simple goal-based AI ecology (people or convoys going from A to B). Open access to more interiors, not only in missions. Provide a tougher tether at some point in the game. The ability to decide where you set main camp/respawn point would be nice.
Also, I wouldn't say no to an optional first person view. That might sound stupid, but there's a pretty nice mod doing that and I find myself enjoying it a lot, especially while shooting and driving, and even during parachute trips.
I don't care for MP obv.
