No, he's actually looking for games with a medieval theme. Fantasy is a kind of approach to realism, simply put. You can have a realistic medieval game, a fantasy medieval game, and even a science-fiction medieval game, unlikely as it may seem. Or you might have a fantasy game set in 21st century Warsaw.
It could be argued as to how many anachronisms are allowed in a "medieval" setting, because there aren't many games that recreate medieval social structure, for example (Darklands comes to mind). However, there's a certain popular consensus as to which parts of the medieval stereotype are essential to the theme, and it seems that the social stuff isn't.

I'm half-joking here, but really. It's like someone asked for a realistic World War 2 game, and gave Return to Castle Wolfenstein as an example, and then people would follow up with Jedi Knight, since there's a war and the stormtroopers carry guns that sort of look like stens.

Depending on where you're drawing the line between medieval and fantasy, you might want to investigate Ultima IX: Ascension, although it's getting a bit long in the tooth these days and personally, I hated it :-)
Rune might be worth a look, but it's low on the RPG content.

I'm an Ultima nut (got a dragon name and everything parked somewhere) but I'm not sure I'd wish UIX on anyone who wasn't a fanatic.

I'm curious, why all the Ultima IX hate? Is it story related, or connected to gameplay, or...? An honest question, it's the only one of the series that I played (not counting the handful of unfruitful hours I put into Pagan, which was after I'd played Ascension), and I loved it to bits at the time. And maybe I'm wrong, but I can't shake the feeling that Gothic, another game I'm very fond of, took quite a lot after it.
