You won't be able to get 100% of the loot in the cities in Thief 3 because you can't get it from the citizens walking around without either scaring their valuables off them or blackjacking them and knocking it off. But luckily that loot doesn't count in your missions so it wouldn't be counted against your loot percentages.
I have not been ghosting my first run through Thief 3. I want to learn where all the objectives are, loot, and special items are first. My second time around, I'll increase the difficulty to expert (hopefully we'll have a patch by than for the bug), and semi ghost to learn the game on the expert mode. My third time through, I will officially ghost the missions. I will not alert a guard above, "was that a rat?" I will get one hundred percent of the loot, and all three special items. I will not use a flashbomb, noisemaker, or water arrow (obviously i can't use the blackjack, broadhead, gas, moss, or fire arrows). I am going to learn all the ai patrol points so that I can make timed entrances/exits. I will not save unless the mission is over. On the city streets, I will not rob the citizens or alert the city watch. I will not buy or sell my loot at the fences (I have no need to, I don't use the items anyways). Once I can beat each level like I know the back of my hand (like I know all the Thievery maps, and can ghost each map in this similar fashion) I will make fraps videos for all to enjoy :) If anybody else is able to do what I've mentioned above, make videos so we can enjoy!!! :)

It's spelled 'poltergeist'. Dictionary definition is 'A ghost that manifests itself by noises, rappings, and the creation of disorder.' Hmm. Not a bad idea to name specific ghost variations, though. Someone ought to get on that.

Most games, even stealth games, simply aren't designed to allow you to ghost. Take the Splinter Cell games, for example. There are several occasions on which you are simply forced to walk up and knock a guy out. With no alternative. Despite the fact that you might simply be trying to climb over a wall, or infiltrate a submarine, merely trying to evade the person (he is not a mission critical objective). There is no *reason* for you having to knock them out, it's just that the levels weren't designed with the imagination to allow more than one way of doing things. That is a big detractor from Splinter Cell, I feel, and for this reason the games I cite below are better stealth games.
The only games (havent played Thief 3) I can think of which seem to have been designed with ghosting in mind as a possibility are Thief 1, Thief 2, and oddly enough, Metal Gear Solid 2. Of those 3, MGS2 probably has the most challenging stealth situations, closely followed by Thief 1, and then Thief 2.
Honourable mentions for purist stealth gameplay are the Tenchu games, and Hidden and Dangerous 2.

Unfortunately, Thief 2 has two missions that can't be ghosted. Notably, Kidnap, and Running Interference on (hard or?) Expert. And I believe that Thief 3 was designed to be ghostable, though it's hard to say for sure until someone confirms that they've ghosted the game.

As soon as the patch is out i will be most happy to restart the game and try ghosting it. Concerning loot. In the Hex Madness post we have now finally an option to turn on 100% loot needed.

There is one mission in Thief III that cannot be ghosted - I think - the one in wich you must get the Jacknall's Paw. You need some blood to reveal the Jacknall's Paw. I couldn't find any other source of blood than a pagan's veins.

There's one pagan there who's already dead.
