Depending on the mission being played, some simply cannot be ghosted at the "Supreme" level (example, Soulforge. Doesn't matter that you start the level already detected and you can't help it. it's a bust. Cannot be Supreme-ghosted.).
People argue about what a 'true' mission ghost is all the time, but that's easy to handle. Just keep this mentality.

One thing that has always bugged me about ghosting Baffords..
"If a door was closed when you started the level, it should be closed when the level is finished."
How to you close the sewer portcullis behind you? Ive never managed to frob the switch and get past before the portcullis blocks the tunnel.

Choose a different method of exit (i.e., if you're down there to get the loot hidden in the water, don't bother opening the portcullis. Just go back the way you came and work your way to the well-house through the city).

I had a problem with this too until I realized I could actually frob the portcullis itself. Simply open the portcullis using the switch, pass underneath it, look up, see that it highlights, and pull it down.
