In the full trailer it actually shows a shot of a helicopter hovering outside the building while Max stands inside looking out, followed closely by a shot behind Max looking out of a window and seeing the valkyries before the windows explode. It's pretty definite that they're hallucinations.

The best action movies in the world want to know who's even approaching them in terms of quality when it comes to PG-13.
Die Hard = R
Predator = R
Robocop = R
Aliens = R
T2 = R
Why is R rating necessary to be good? Well, it depends on whether you want your action to be visceral and have any realistic impact on the people around it... you know, action tends to be destructive, violent, bloody and explosive, any proximity to those things usually incurs swearing, bleeding and occasional dismemberment.

OK, so I found the script. Looks legit, everything matches up with all of the trailers.
About the demons:
Valkyr is described as creating exactly this type of vision. It's "rationally" explained too. Max gets drugged by someone near the beginning of the movie without knowing it. That's on the night where the window scene happens.
About Max taking valkyr:
He takes valkyr consciously once, before the final showdown. Otherwise he'd be dead - he just got out of that freezing river, he's also injured and bleeding
Basically, all is ok.

A case can be made that LotR, Indy, and Jurassic Park don't count as "Action Movies," but they all have action sequences that are every bit as exciting (and in the case of LotR every bit as bloody and visceral) as in the films you listed.

Pah. Only Bourne comes close to being categorised as an action flick and even then, Bruce Willis with wrapped up bloody feet and a dirty wifebeater would still find a way to kick that amnesiac's ass and wrap it up with a swearword-laden one liner.

Casino Royale is a Bond flick, isn't it? Aren't Bond flicks typically PG-13 at most anyway?
