Dude, there's like a giant paragraph on that page explaining exactly what the Sandvich does. It's an item that goes into one of three weapon slots. The heavy eats it and gains health.

Okay, yes, bad choice of word. The Heavy seemed to me to be one of the East European ethnic groups, and Russian is a little odd.
