I like the moderator approach, although I also think maybe all the regulars (or at least TTLG people) should be able to get op status fairly quickly.
Koki, you have to log into minecraft.net in order to join a multiplayer game. It's impossible afaik to spoof someone's nick, at least in an obvious way.
I think the best aspect of the moderator approach is that it still lets anyone log into the server to see people's things, it just doesn't let them do anything destructive. Seems like a good balance to me.
Another good aspect of the moderator approach is, should someone be given build access and start going batshit crazy, it should be easy for an op to demote them quickly.

As far as I know, by default, people are authenticated via their minecraft.net account whenever they log in.
