It's a purely subjective feeling- it was the first game I played after a 6 years voluntary break (I just lost interest)- the leap in technology really blew me away- the gfx.... the physics.. I know all these things are considered superficial here, but they sure help you forget you're playing a video game..
Not to mention the cohesive structure of the story (aside from few chapters).
And sometimes (well.. rarely) it only takes one or two innovative/ extremely-polished elements to make me lose myself in a game.
Take Mass Effect (1), for example- the story=nothing special, RPG elements=some good, most are poorly implemented, combat=meh... but in the department of digital acting it's unsuppressed, and that's how I was sold to the franchise- seeing my custom character come alive.
Both ME, HL franchises doesn't have a mind blowing lore/world/story- nothing I hadn't already read or saw in films- but the ways in which they utilize their medium to deliver it, are brilliant.
Maybe the perfect game = VTB:M story & dialogs + ME production values + Bethesda huge, free roam world...

Not superficial in my opinion, the graphics, art style, and physics were HUGE parts of what helped HL2 set the bar so high.

Could a freeform Bethesda style world actually have Bloodlines level dialogue & story though? Trying to keep story cohesion while allowing vastly freeform worlds has never gone real well.

A potential solution to that is to railroad the game into a mission structure, like Mafia did. You've got the freedom to roam around the city in most missions, but you've still got a primary goal that needs doing. It's an odd approach, but it worked in Mafia.
