Depends entirely on where you live. There are countries even today were surnames still don't really exist; I believe in Iceland, for example, people are still called "son/daughter of such-and-such".

There are actually a lot of last names in the fiction, but it's not consistent. Some people have only one name (either because we are never told their first / last name, or maybe because they haven't got one -- certainly in the Real World that comes about because the NPC in question is too minor to warrant a full name, Kevel I'm looking at you), some are known by an epithet (Basso the Boxman, for instance), and some have two names (just wander around Shipping / Receiving a while, you'll see).
I've always thought that the mish-mash was good. It lent more weight to the social stratification, religious and otherwise dogmatic ideologies, and general "fend for yourself" nature of the City.
I don't imagine for a second that it was a deliberate effort on the part of the writers to create such a commentary, but it's just another one of those things which -- when taken alongside all the other threads and undercurrents and asides and cryptic references -- combine to give the first two Thief games one of the richest gameworlds outside of an RPG. The secret, I think, is to hint and refer to things rather than spell it all out.

Wasn't Garrett a street scamp, thus, probably an orphan or abandoned child. Wouldn't Garrett be rather lacking of the proper channels for a surname? I say Garrett is his only name unless he had decided he wanted one.

My thoughts exactly... He has no family, and lives outside of the law. He wouldn't have, or need, a last name. =P
