I've found there isn't really much of a standard. Depending on the game the functions are all different.
If i'm playing Call of Duty, ADS is right mouse, whereas in Half life its secondary fire.
Melee is left alt, hit it with my thumb, unless there is no sprint function in the game, in which case I might map it to shift.
Grenades are usualy MMB (which means scroll wheel click) unless I have a secondary fire AND ADS in the game i'm playing, in which case I use G
stuff that never changes if I can help it:
F = "activate/pickup/examine/etc."
E = flashlight
Space = jump.
Tab = Inventory / menu (if different from the ESC menu)
Ctrl = crouch
Z = Prone if there is a prone, and if not (FEAR) then its medkit/healing
I use WASD for movement, but I have a friend who never uses it, if he can he remaps it to ESDF so that if he is in a twitch fest (UT99) and he has scratch is cheek or something he can just flop his hand back down on the keyboard and find the F key without looking.
Also, none of this takes into account mice with more than 2 buttons and scroll wheel and/or the ability to program macros.

When it comes to PC gaming I play FPS-style games almost exclusively. As far as movement goes my controls differ somewhat from the "norm".
Forward: right mouse button
Backwards: M
Left: , (comma)
Right: . (period)
Jump: Space
Crouch: C
Walk: Left Shift
That way, the three middle fingers of my left hand rest on the back/left/right keys, with my little finger on jump. It doesn't take much to temporarily move the hand to any other keys I need.
Fire is left mouse, alt-fire (if there is one) is middle mouse.
For any other controls I'll usually try to match a letter to the action, so Reload is R, Flashlight is F, Grenades if G if there's a separate grenade button, U is Use (although I usually have middle mouse as the Use button if there's no alt-fire option in that particular game.)
It's a little different but it works for me.

I've been using these controls since i played Quake in the 90s. :)
A - Forward
Z - Back
X - Strafe left
C - Strafe right
S - Jump
D - Crouch
Space - Use
R - Reload
F - Flashlight/Night vision/whatever
LALT - Melee/Special
Q - Lean forward/Autorun etc.
W - Lean left
E - Lean right
Shift - Run
Tab - Some kind of in-game menu(where applicable)
LCtrl - Walk/Sneak
F5 - quicksave
F12 - quickload
Inverted mouse
