I remember seeing the boss of Serious Sam for the first time. That REALLY made me go, "Oh crap!"
Serious Sam, both, in general, are "oh, shit." games. i_i AAAAAAAAAAAAAA!
Also, some of the pre-battle cutscenes in Metal Gear Solid (Rex, M1, Hind. 'nuff said).
Now, you shall bear witness to the demon weapon that will drag the world into the twenty-first century! <3
Finally, the last part in Ocarina of Time
when Ganondorf transforms into his monstrous alter ego and emerges from the remains of his castle
.

Quicksaving after killing every enemy on the Haunted Cathedral level, going up some stairs and then getting gibbed while stuck going up the stairs. I quick saved 1.3 seconds before Garrett died.
Did something just as messed up on that mission. I cleared out all the dead inside the Cathedral only to discover I'd have a tougher time outside with all my mines and water arrows used up. :erg:

I have a natural talent for quicksaving milliseconds before I die...but, because I'm so stubborn, I keep reloading and reloading until I find that perfect combination that'll let me live.
Ikaruga, Chapter 3.
Half Life, Tentacle boss.
Resident Evil 0, the second Tyrant.
Resident Evil Remake: the Tyrant on the helipad.

Dropping an ESSENTIAL ESSENTIAL item through the world in Hythloth dungeon, well into Ultima IX, PC. OOPS. And forgetting how I solved the end of last boss area at last savepoint.
WITH fab experience, weapons and, oh EVERYTHING, dammit.
I almost never quicksave; it's useless for me, except in RTS games where I will die worse anyway. Starcraft.

That's the exact reason why I never finished System Shock 2.
Same Here!

Just playing Hidden & Dangerous 2. Killed all but one AI on a map only to find I'd shot the driver whose uniform I needed unsullied. Killed all AI on another map only to find I needed six explosives packs, not the 'recommended' three.

Retro OH SHIT!!! Moments:
Doom: First time encountering a cyberdemon. Dead 3 seconds later.
Wolfenstein: First encounter with Adolf Hitler. Dead shortly after.
Quake: Watching Blahbalicious for the 1st time (Quake movie). Funniest fan made game movie I`ve ever seen.
Modern OH SHIT!! Moments:
Far Cry: Coming down an elevator to be confronted by two missle launcer armed Trigens. Took me a fair few tries before I worked out a way around them.
Doom3: Reaction after trying to run it on my computer and having to give up in disgust after 5 minutes.

Playing Medieval: Total War as the English I attacked Corsica and Sardinia which were held by the Sicilians. On the next turn Sicily retaliated by attacking Morrocco with 7,000 troups against my garrison of around 750! Out numbered nearly 10:1 I fought of the first 2 waves of attacks before withdrawing my exhasted troops in an orderly fashion to the castle having taken only minimal losses.
It didn't go so well after that. I had to move most of my troops in Spain to Morroco (to keep my hold on Africa) while frantically producing more troops in Navarre and Aragon (to hold the Sicilians back from France and, ultimately my homelands). End result: I lost most of Spain. Also, whilst my island assault force had captured Sardinia and Corsica they were stranded as Sicily had a superior naval force in the Mediterranean.
So more of an "Oh shit" moment that didn't actually turn out that bad.
(Blackjack, if that's the lighthouse mission you're talking about you can find more explosives in the central (?) cellar area.)
