Yeah, people can get away with using heroin long term with little to no consequence, provided they get a good quality regular supply. It's one of the issues with criminalisation - a lot of the issues surround drug addiction (crime, ill health) are as a result of the criminalisation to a large degree.
But to say that the danger of heroin is overblown, especially with current laws and their effect on price and purity, is misleading.

I don't have any dogmatic view on the subject, and I think each drug should be looked at case by case. Personally, once in my life I want to try a hallucinogen like LSD, with a doctor & my family nearby and in a sense-deprivation tank since apparently being on it in that thing is supposed to be some kind of mad spiritual experience, and I want to treat it like a big deal, with a kind of reverence... And then just keep the memory of that one time.
But my general view is... even before I got to Asia, I believe the best kind of life (for me) is one where I don't go to extreme highs or lows but coast in the middle, zen or stoic like. So the idea of "more intensity" doesn't really appeal to me. Drinking and the occasional flavored tobacco hooka set me up just fine.

Yeah, a lot of David Nutt's stuff is a trifle cherrypicked. Alcohol is pretty bad less because it's inherently bad, and more because it's FUCKING EVERYWHERE.
Still, his reports are usually a fuckton better (i.e. based on actual data) than whatever craziness informs government policy, though.

If as many people used heroin as used alcohol we'd soon get the population down. The window of safety is much smaller.
