It depends. You can only use VATS as long as you've got so many Action Points. If you're flanked by 3 high-level enemies, you might have enough AP to take one out with VATS, but not the other two. If the play's really laggy at this point, you're pretty much hosed because your aim's buggered.
There are ways to get around it, of course. You can run away and circle around to get more AP, which can be a rather cheap combat tactic, but there you go.
