My guess is that the cameras and security bots are programmed to recognise the servant's as being friends. This makes sense since they can be recalled for adjustment at any time and having the security systems make mincemeat out of them wouldn't be helpful.
Karras isn't actually sure where Garrett is at any given time. You don't even have to set spotted by the camera at the start of the level if you are quick enough. If you get spotted by cameras during the mission Karras will even indicate he is surprised that Garrett is still there.
Has anyone actually confirmed in the game code/scripts that the "dont let Karras know whats going on" objective can't be failed or are we assuming based on never failing it. If it could be failed I'd guess it would be due to getting spotted by too many cameras or cameras in too many locations.

All the other missions seem to use the cameras as sound alarms only - no video. Otherwise there would be video feedback at the booths that control the cameras (especially in Precious Cargo and LotP). I've always assumed Karras' VO traps being near cameras as a coincidence, not an indication that he's watching Garrett the entire time.

There's two different sets of speeches that he gives. The "regular" speeches seem to happen at regular intervals regardless of whether there's even a face nearby, and are apparently coming out of every speaker in the cathedral at the same time (you can run from room to room during a speech to verify this). The "Still here?" speeches are triggered whenever you set off an alarm. I'm inclined to think that he can't see you, but likes hearing the sound of his own voice so much that he can't resist talking to you periodically.

that funny voice disorder added much to his character imo. he said lots of funny stuff. thats why i like him.
