Well, there are lots of FM's that have creeped me out. However, theres only been one where ive actually made an audible yelp and that was in Deceptive Perceptions. The bit at the very end nearly gave me a heart attack.
As far as OM's go, yeah have to agree with most people that The Cradle was the creepiest for me personally.

The sound effects of the Zombies and the Haunts alone is enough to send chills down my spine but combine them with dark levels: The Cathedral, Old Quarter and Bonehoard - and you've got a recipe for giving anyone the heebie jeebies. :)
Oh and I couldn't count myself as a horror aficionado if I didn't mention the Cradle being one of the creepiest thief missions to date.

The cradle was so much easier for me when I turned off the sound. The background noise and the miscellaneous clicks and whistles really made it freaky for me. The puppets were just a toss in to give it a physical danger.
