Which enemy freaked you out or scared you the most in a game? I know many people will say the Haunts from Thief are the scariest, but what about the headcrabs from Half-life or the slimers from Duke Nukem 3D? I think small enemies that sneak up on the player and surprise them are scarier than tough enemies that give the player warnings before attacking. My vote goes to the slimers for this reason.

The Puppets from Thief: Deadly Shadows.
IMHO they were the worst I ever faced.
Runners Up:
- Facehuggers from AVP2 (specially the "death scene")
- Fast Zombies on HL2 (they make noises before attacking, but it´s fearful noises)
I still have to play Penumbra, AVP2 Mod> Island of Fear, HL Mod> Scared of Monsters, and Call of Cthulhu: Dark Corners of Earth. From what I heard these ones may contain the good stuff for this list.

For whatever reason Morrowind's various sixth house cultists especially the Ascended sleepers creeped me out. I had the house to myself when Morrowind first came out, and I swear I lived in that game for about a week.

While I certainly got freaked out by The Cradle etc and little creepy buggers are usually more effective, sometimes an enormous monster with a monstrous roar can still scare me. The first time I encountered the Cyberdemon in Doom was pretty scary, simply because you can always hear his roaring or mechanical stomping wherever you go in that level. It felt like he was searching for me and at any moment he was going to appear at the entrance of my hiding place. This was during my early gaming days so me lack of skill made me feel even more vulnerable.

The insecty things that jumped on your face and filled the viewport completely in Corporation. Banged my knee on the table very painfully when the first one sprang on me.

The shoggoth from Call of Cthulhu: Dark Corners of the Earth. Nothing else even comes close. That thing actually had me physically lift my arm away from the keyboard to protect my face, just before it killed me in-game.

Funnily enough, that's exactly what the slimers do in Duke3d. Sometimes they come from the top down too.

The black creeping headcrabs in HL2. This first time I encountered them I had a proper panic, especially that bit where there two at the bottom of the stairs and you can see them and you know THEY ARE COMING FOR YOU.

Getting attacked by the giant worm in Penumbra: Overture was a pretty huge OHSHI moment, and of course anything that jumps at your face is good for a jolt.
