I have to say those fast, wall-climbing, ninja things from FEAR freaked me out quite a bit when they first showed up. I completely forgot my slo-mo and just started firing wildly around with the shotgun.
Also, those demon-dog things from Clive Barker's Undying were kind of freaky the first couple times. Of course, then the game kept throwing them at you over and over again to the point where they just got annoying.
Oh, and how could I forget the Midwives from System Shock 2? I seriously dreaded going into some areas where I could hear their weird electronic voices mumbling to themselves. *shudders*

I've gotten killing them entirely by melee down to a science, especially towards the end when they jump you in the abandoned industrial complex. It takes good timing and clever use of slo-mo but those fuckers get schooled with a boot to the face.

Yea I would have to agree about the ninja soldiers or whatever they're called. Although I did get freaked out more by those stealthy ghost creatures from FEAR:Extraction Point. They moved really fast and had a hit & run strategy and you could only really kill them with Slo-Mo. Here's a pic of the creatures I'm referring to(with Alma in the background):

The monkeys in SS2. I still get nervous when I hear a monkey sound like the ones they make. Also, the little spider bots in the original Deus Ex. In my first playthrough my character was totally unsuited to fighting them (an assault rifle will eventually destroy them, lol) and it scarred me for life.

I start panicking whenever I hear the roars of a Tank nearby in Left 4 Dead. Just because those fuckers are usually instant death and I don't have the luxury of quickload.

Are we talking about legitimately giving you nightmares or just enemies that you freak out and dread when you see?
Because I think the enemy runners in Mirror's Edge did a pretty good job on the second front. I was calm and collected thruout all of that game but when those guys showed up all hell broke loose and I went full panic.
