Archies are especially frightening in Plutonium MAP11 ("Hunted"). There are only 11 monsters. They are all Archviles. And you're in a maze with them.

The FEAR creatures Andarthiel and ZymeAddict mentioned screwed with me pretty bad the first time I encountered them and they kept putting me on mega-alert mode whenever I'd have to deal with them in Extraction Point and Perseus Mandate. I think in FEAR 2 they got upgraded into those ghostly things that first appear in the ruins of the city and later in the school. Me and the shotgun became so intimate afterward.

Not really scary but kamikazes from Serious Sam TFE and TSE made me panic and gave big shock from time to time. Especially when suddenly one appeared from nowhere and you were in tight corridor. Oh God, I can still hear their loud shout. :p

I'm not extraordinarily scared by "piles of goo" like Cthulhu's Shoggoth, neither by classical horror properties - skeletons, zombies, ghosts... I'm more disturbed by "dehumanized," techno-genetically altered people - SS2 hybrids and midwives, insane disfigured childred, that sort of thing.
Actually everything depends on many dramatical factors, storytelling and how the game presents it's monsters. If a gothic horror presents it's Mummy in the right way and I, as player, am being pushed into corner by the monster, have limited opportunities how to get rid of the threat or even have to flee to save myself, that's good.
