Most anything from Silent Hill 2. That is one freaky game to this day.

Don't expect many remember that, but what a heart attack! Just goes to show that timing (Or sound in the case of SS2 monkeys) can beat gruesome graphics.
My fav - the 'children' in the school in Silent Hill 1.
Also - the sea monster that gets you when you go out of bounds in Gothic 2

Fuck, yes! Especially the little girls with just their arms and legs poking out of a grubby sack, crying as they attack you.

Heh, it bloody terrified me; the adrenaline rush and feeling of "buggerbuggerbugger, I've got to get away from the thing AIEEEEEE!" was funny as hell.
Quite surprised even one person got the reference though :)
As for modern day, those bloody fishmen in Call of Cthulhu. Again, the rooftop escape sequence was frantically charged with a panic rarely induced by other games.
Bastards.
Side note: A friend who lurks here had to play the Cradle with the sound off, it scared him so much :D
